About the PREVENT equations

PREVENT gives 10-year ASCVD risk from a sex-specific logistic model whose predictors are centred and spline-transformed: age per 10 years, non-HDL and HDL cholesterol in mmol/L, systolic blood pressure split at 110 mmHg, diabetes, smoking, eGFR split at 60, and treatment for blood pressure and statins, plus interaction terms. What are the PREVENT equations?

PREVENT (Predicting Risk of cardiovascular disease EVENTs) is a set of risk equations released by the American Heart Association in 2023 to estimate cardiovascular risk. They are sex-specific and, unlike the 2013 Pooled Cohort Equations, do not use race. They also add kidney function (eGFR) to the standard risk factors and were derived and validated in a large, contemporary and diverse US population. This tool computes the base 10-year risk of atherosclerotic cardiovascular disease (heart attack, coronary death, and fatal or non-fatal stroke).

How is PREVENT different from the older ASCVD calculator?

PREVENT differs from the 2013 Pooled Cohort Equations, which were race-based and tended to overestimate risk in current practice. PREVENT removes race, is sex-specific, adds eGFR (kidney function), applies to a wider age range (30–79 rather than 40–79), and was fitted to more recent data — so for many people it returns a lower, better-calibrated estimate. Our ASCVD calculator still reproduces the 2013 equations, which some guidelines continue to reference; PREVENT is the newer approach.

What counts as high risk?

PREVENT reports a 10-year percentage. Treatment thresholds specific to PREVENT are still being defined by guideline committees, so this tool shows the familiar categories for context: under 5% low, 5 to under 7.5% borderline, 7.5 to under 20% intermediate, and 20% or more high. Because PREVENT is generally better calibrated, the same percentage may carry different treatment implications than under the older equations — discuss decisions with a clinician.

Where do I get my eGFR?

eGFR (estimated glomerular filtration rate) comes from a blood creatinine test and is often reported alongside it. If you have your creatinine, age and sex you can estimate it with our eGFR calculator, which uses the race-free CKD-EPI 2021 equation. A normal value is around 90–120 mL/min/1.73m²; PREVENT accepts values from 15 to 140.

Who is PREVENT valid for?

The base PREVENT equations apply to adults aged 30–79 without known cardiovascular disease, with total and HDL cholesterol, systolic blood pressure, treatment status, diabetes, smoking and eGFR. Extended versions can add HbA1c, urine albumin-to-creatinine ratio and a social-deprivation index, but the base model — used here — needs none of those.
