About the ANC
The absolute neutrophil count converts the white-cell count and differential into the actual number of neutrophils, the main defence against bacterial and fungal infection, and is the standard basis for grading neutropenia (for example the CTCAE grades).

What is the ANC formula?
ANC = white-cell count × (percentage of segmented neutrophils + percentage of band neutrophils) ÷ 100. With the white-cell count in ×10⁹/L this gives the ANC in ×10⁹/L; multiply by 1000 for cells per microlitre. For example, a white-cell count of 5.0 ×10⁹/L with 60% segs and 3% bands gives 5.0 × 63 ÷ 100 = 3.15 ×10⁹/L, or 3150 /µL.
What ANC counts as neutropenia?
Using cells per microlitre: 1500 or above is normal; 1000–1499 is mild neutropenia; 500–999 is moderate; and under 500 is severe neutropenia, which carries a high risk of serious infection. An ANC under 500 (or expected to fall below it) defines the neutropenic state that, with fever, is a medical emergency — neutropenic sepsis. The same bands in ×10⁹/L are 1.5 or above normal, 1.0–1.49 mild, 0.5–0.99 moderate and under 0.5 severe; this calculator shows the result in both units. Infection risk also depends on how long the count stays low and how fast it is falling, not on a single reading alone.
Why include band neutrophils?
Bands are immature neutrophils that still contribute to fighting infection, so they are included in the ANC alongside the mature segmented neutrophils. Some laboratories report only a combined “neutrophil” percentage, in which case you can leave the band field at zero and enter that single figure. Other early forms such as metamyelocytes are usually not counted.
When is the ANC used?
The ANC guides care in chemotherapy and other causes of low white cells — deciding on isolation, growth-factor support, prophylactic or urgent antibiotics, and whether it is safe to give the next cycle of treatment. A febrile patient with a low ANC needs prompt assessment and antibiotics. It is also used to grade drug-induced and congenital neutropenias.
What units should I use?
